Tier II reporting requirements: what the form needs, field by field
A Tier II report has two parts: facility information (identifiers, owner/operator and emergency contacts) and an entry for each reportable chemical (identity, hazard categories, maximum and average amounts as range codes, days on site, storage type, conditions and location). The owner/operator or a designated representative must certify it. The required elements are listed in 40 CFR 370.42.

- Reporting year, and whether page one is identical to last year's
- Full facility address, latitude and longitude, manned or unmanned, and maximum number of occupants
- NAICS code, Dun & Bradstreet number, and TRI and Risk Management Program IDs (or N/A)
- Whether the facility is subject to EPCRA section 302 emergency planning and to the Clean Air Act 112(r) Risk Management Program
- Owner/operator name, address, phone and email; parent company (optional)
- Facility emergency coordinator if subject to section 302, the Tier II information contact, and at least one emergency contact with a 24-hour phone number

For each chemical or mixture
- Chemical or common name and CAS number as on the SDS (a mixture uses its product name; check "Not Available" if there is no CAS number)
- Physical state (solid, liquid, gas), whether it is an EHS, and the EHS components in a mixture
- Hazard categories, taken from section 2 of the SDS
- Maximum amount on any single day and average daily amount, as range codes
- Maximum number of days on site
- Storage type (above-ground tank, steel drum, cylinder and so on), storage conditions (pressure and temperature) and precise location

Hazard categories
Tier II now uses the physical and health hazard classes from SDS section 2. Examples: flammable liquids, gases under pressure, corrosive to metal, acute toxicity, skin corrosion or irritation, carcinogenicity, simple asphyxiant, and "hazard not otherwise classified" (40 CFR 370.3).
Confidential location and trade secrets
You may ask that the location of a specific chemical be withheld from the public. You still give it to the SERC, LEPC and fire department, on a separate Confidential Location Information Sheet (40 CFR 370.42(s)(12), 370.64(b)). Withholding a chemical's identity as a trade secret requires a substantiation filed with EPA under 40 CFR part 350.
Certification
The owner/operator or officially designated representative certifies under penalty of law that the information is true, accurate and complete. Many states require electronic certification in their portal (40 CFR 370.42(a)). A preparer can build the report, but the certification belongs to the facility.
Requirements by state
States can require more: extra chemicals, lower thresholds, fees, site plans or a specific portal. EPA links each state's rules on its state requirements page.
